Chapter 18 PAIN AND FRUSTRATION

Chapter 18 PAIN AND FRUSTRATION
Luciano
The ambulance siren is a screaming, panicked blur, but it can't drown out the sound of Annabella's pain. My hands are coated in the fresh, metallic scent of Killian’s blood. He’s on the gurney beside us, his chest bandaged, his breathing shallow, a stark contrast to the guttural, raw effort Annabella is making on her own stretcher.
Focus. One foot in front of the other. That’s all I can think. Adrenaline is a poison, but right now, it’s my only fuel. I watch the monitor beside Killian, watching his vitals drop, watching his face go whiter, thinner. Is this it? Is this the price for peace?
"Breathe, Annabella, you have to breathe!" I shout over the siren, squeezing her hand so hard I must be bruising her.
She is thrashing, clutching her swollen belly, her face a mask of terror and agony. "I can't, Luciano! I can't! It hurts! Where is he? I need Killian! I need him!"
"He's right here!" I lie, watching Killian’s lifeless form. "He's fighting, Annabella. And you have to fight, too. For him. You hear me? This is the Astor heir. You bring him into the world now."
The ambulance doors fly open. We hit the blinding, sterile chaos of the Emergency Room. A swarm of white coats descends.
"GSW, trauma, massive blood loss! Get him to OR 3!" a deep voice barks.
Killian is wheeled away in a terrifying instant. I try to follow, but a nurse grabs my arm, her grip surprisingly firm.
"Sir! She's crowning! This way! L&D, stat!"
I am ripped from my focus on Killian and shoved toward Labor and Delivery. I don't belong here. I belong in a fight, in a boardroom, or in a dark corner protecting the perimeter. I don't belong in a brightly lit room filled with soft blankets and birthing equipment.
I pace outside her room as they prep her. My phone vibrates, Alex.
"Luciano? What the hell happened? Killian's on the table? Thorne?"
"Thorne is dead. Killed him myself. Explosion went off, minimal damage, but Killian took a bullet. Annabella... she's in labor," I rasp, running a hand through my hair.
"Labor? Now? Jesus. I'm on my way. I'll handle the police and the press. You focus on them. You're the only one there."
The nurse sticks her head out. "Mr. Luciano? She's asking for you. She's dilated eight centimeters. No time for the father, you're it."
You're it. That's been my life for weeks, hasn't it? The convenient stand-in. The faithful shadow.
I walk into the room. Annabella’s scream is a pure, visceral sound.
"Luciano!" she gasps, her eyes wide and pleading. "Don't leave me! Please!"
I rush to her side, taking the chair Killian should be in. I take her hand and lift her sweat-drenched hair off her forehead. "I'm not going anywhere, Annabella. I promise. I'm right here."
A powerful contraction hits, and she arches her back, screaming Killian's name. "Killian! I need him! It hurts so much!"
"Look at me, Annabella! Look at me!" I demand, gripping her chin gently, forcing her focus onto my face. "He took a bullet for you. He gave you a piece of his life. You have to honor that! You honor that by bringing his son or daughter into this world! Stop looking for him and push!"
She lets out a primal roar, pushing down with every ounce of strength she has. It's magnificent and agonizing to watch. The pain in her eyes is mirrored by the pain in my own heart. I held her hand while he was shot, and I am holding her hand now as she gives birth to his son. My sacrifice is complete, but damn it, I didn't want to lose him, too.
I love her. I gave her up. I stood in the darkness and shot the man who was going to take her life, securing her freedom at the cost of his own life. I knew I couldn't win her heart, but I can protect her life. I will be the godfather, the guardian, the silent partner in this twisted family. I will raise Killian's son if Killian can't. This is my burden. This is my love.
"Just a little more, Mrs. Astor! I can see the head! Strong push!" the doctor encourages.
Annabella is shaking, exhausted, her voice hoarse. "I can't push anymore! I'm tired!"
"Yes, you can!" I shout, my voice cracking. "You are the strongest person I know! He needs you! The baby needs you! Push, Annabella! Push like you want to kill me!"
She lets out a final, agonizing cry, and then, suddenly, there is only a gurgling, wet sound, followed by a thin, furious wail.
The doctor holds up a small, purple, squalling bundle. "It's a boy! Congratulations!"
I slump back in the chair, releasing the breath I didn't realize I'd been holding for seven days. Annabella is sobbing—sobs of relief, not pain. The nurse cleans him up and places him on Annabella's chest.
"Oh... Killian..." she whispers, her eyes fixed on the baby's tiny, perfect face. "He looks just like him."
The peace lasts for maybe five minutes. A harsh knock on the door shatters the sacred silence. A doctor from the surgical wing stands there, his face grave.
I immediately walk to the door, shielding Annabella and the baby. "What is it? Killian Astor."
"Mr. Astor is out of surgery," the doctor says, his voice low. "The bullet missed the heart, but the impact caused extensive internal damage and significant blood loss. We stabilized him, but he sustained trauma... we found some swelling on the brain. We have placed him in a medically induced coma. We won't know the extent of the neurological damage until we bring him out."
My blood runs cold. "Coma? You mean... he might not wake up?"
"We are doing everything we can, Mr....?"
"Luciano. I'm... his business partner." I can barely get the words out. "And the baby?"
"The baby is healthy. The mother is exhausted but strong. She needs to rest now."
I nod numbly. The world has shifted again. Joy and devastation, existing in the same tiny, bright room.
I wait until the sun rises, until the morning shift nurses come in, until Annabella finally falls into a deep, drug-induced sleep, her son nestled against her. I pull a chair next to the crib and stare at the baby. He has the Astor dark hair and the Astor jawline, already defined. Killian's legacy. My burden.
Annabella wakes up an hour later, her eyes immediately finding the crib, then me.
"Luciano?" she whispers, her voice raspy. "How long?"
"Just a few hours. You have a beautiful son, Annabella."
She smiles, a fragile, exhausted expression of pure maternal love. "Where is Killian? Did he see him? Tell him to come in. I want him to hold his son."
This is the hardest conversation of my life. I take a deep breath, walking over to her bedside, taking her hand. It's cold and damp.
"He can't, Annabella. Not yet."
"Why? Is he angry about the file? Tell him I don't care about his father anymore. Tell him he took a bullet for me, that wipes the slate clean."
"He knows you love him," I say gently. "He told me. But he needs time, Annabella. A lot of time. The surgery was successful, but..." I hesitate, searching for the softest words to deliver the hammer blow.
"But what, Luciano? Stop treating me like I'm made of glass. I just had a baby. Tell me the truth." Her eyes are wide now, filled with a sudden, dawning terror.
"He's in a coma, Annabella. The bullet caused massive trauma and blood loss. They induced the coma to keep the swelling down. They don't know when he'll wake up. Or... if he will."
She doesn't scream. She doesn't cry. She just stares at the ceiling, one tear tracing a slow, solitary path down her temple. She whispers, a sound so tiny it barely disturbs the air.
"No... no, no, no. He promised me. He said he would protect me. He said he would make sure nothing happened to me."
"He kept that promise," I argue fiercely. "He saved you. He saved your son. He gave you the ultimate protection."
"And now he's gone," she chokes out, turning her head to me, her eyes filling with a torrent of tears. "He saved me, and now he's gone. What am I supposed to do, Luciano? What do I tell my son?"
I lean down, kissing her forehead. "You tell your son that his father is a hero. And you let me help you. I am here, Annabella. I won't leave. We are in this together. Always."
I watch her, the new mother, the devastated lover, and the guardian of the Astor heir. I know my place now. I am the man in the shadows, the perpetual protector, the sentinel guarding the door of the two people I love most. The battle is over, but the war for their future has just begun.
